Completing this form is the first step toward getting your physical library card and key tag card, which can be used at any CLEVNET library and for online services and library apps.
However, if you already have a card from a CLEVNET library -- locally, this means all Lake and Geauga county libraries and most Ashtabula County libraries -- you do not need to apply for a Madison Public Library card. A card in good standing from any CLEVNET library works in Madison.
However, there are some services only Madison Public Library cardholders can access. To switch from a card from another CLEVNET library to a Madison card, let us know. We can make that happen!
